We're seeking highly skilled Clinical Program Coordinator for our Digestive Center located in NYC. The hallmark of this new center is a uniquely integrated approach to care, in which clinical program coordinators guide patients from initial diagnosis through treatment and recovery. You will ensure high quality care by serving as the liaison between attending physicians, social workers, family and other clinicians on the team. The Clinical Program Coordinator is responsible for ensuring optimal continuity of care for assigned Service Line patients and their families through support, coordination, and education of care and fostering collaboration among physicians and the broader interdisciplinary care team participating in clinical activities. Provide health care including clinical assessment and follow up to the assigned patient population.
Full Benefits and Salary. Base salary range is $90-110 K, based on experience.
Doctor Job Requirements
Qualified candidates are Registered Nurses (RN) with a minimum five year's acute care experience and NYS licensure. Candidate must have relevant RN experience.
Preferred candidates will have critical care experience and a master's degree.
